No evidence spinal manipulation helps for non-musculoskeletal disorders

An international group of 50 researchers participated in the Global Summit on the Efficacy and Effectiveness of Spinal Manipulation Therapy (SMT) for the management of non-musculoskeletal disorders. All but six of the researchers agreed with the overall conclusion of the literature review:

*** Our systematic review included six randomized clinical trials (534 participants) of acceptable or high quality investigating the efficacy or effectiveness of SMT for the treatment of non-musculoskeletal disorders. We found no evidence of an effect of SMT for the management of non-musculoskeletal disorders including infantile colic, childhood asthma, hypertension, primary dysmenorrhea, and migraine. This finding challenges the validity of the theory that treating spinal dysfunctions with SMT has a physiological effect on organs and their function. Governments, payers, regulators, educators, and clinicians should consider this evidence when developing policies about the use and reimbursement of SMT for non-musculoskeletal disorders.

Of the participating researchers, 45 had a PhD, had expertise as a chiropractor or in chiropractic research, and were actively involved in research at an academic or educational institution. Three others were chiropractors with a strong publication and scientific editorial record but who did not have a PhD. Two others were early career researchers with expertise in chiropractic and pseudoscience.

Essay calls for countering anti-science attacks on biomedicine

A recent essay by Peter Jay Hotez, an American scientist, pediatrician, and health advocate, compares the White House disinformation efforts in 2020 with the “relentless attacks on science and scientists during Stalin’s Great Purge” and the ascendancy of Lysenkoism and other pseudoscientific theories. The essay faults the Trump administration for

(a) downplaying the severity of COVID-19,

(b) falsely attributing COVID-19 deaths to comorbidities to artificially reduce actual disease mortality rates,

(c) aggressively promoting nonpharmaceutical interventions,

(d) hyping the use of hydroxychloroquine, and

(e) making scientifically unsubstantiated claims about herd immunity. The essay warns that a triumvirate of “medical freedom” initiatives in the U.S., Russian disinformation, and right-wing extremist groups in Western Europe are accelerating mistrust in vaccines and COVID-19 prevention.

The essay also calls for the formation of a U.S. government interagency task force and an expansion of international efforts to dismantle anti-science attacks on biomedicine.

FDA criticized for inadequate protection of consumers from tianeptine.

Consumer Reports (CR) has found “significant shortcomings” in the FDA’s enforcement efforts regarding illegal dietary supplements containing tianeptine

, an opioid-like drug that is classified as a controlled substance in some countries. CR’s articles note:

*** Poison control centers have received 883 tianeptine-related calls since 2015.
*** At least four people have died after using tianeptine.
*** Nearly two dozen users have reported hospitalization and other adverse effects.
*** Records show the FDA is aware of dozens more deaths and adverse effects outside the U.S.
*** Adverse effects tend to be underreported.

The FDA waited at least nine months to alert the public about the risks of tianeptine despite alarming reports of harm in 2017 and 2018.
Consumers Union recommends establishing a dietary-supplement registry to which companies would be required to submit to the FDA a marketed supplement’s name, its ingredients, and the company’s contact information. Such information would enable the FDA to start enforcement actions quickly when illegal ingredients are listed. To achieve this, Congress would need to give the FDA more enforcement authority.
